This position requires independent judgment based on written policy and procedures.
This is an OPS Nutrition Educator position located at the Bushnell/Wildwood WIC offices.
Certifies and recertifies clients for the WIC program including nutrition assessment and assignment of nutrition risk factors based upon procedures found in DHM 150-24 Policy and Procedure Manual.
Prescribes and makes changes in the WIC food packages for clients.
Provides participant centered nutrition counseling/education that is pertinent to the needs of the client.
Provides high risk nutrition care plans for clients enrolled in the WIC Program.
Documents high risk nutrition care plan in the FL-WiSE system using the SOAP (Subjective, Objectives, Assessment and Plan) method.
Provides close follow up to high risk nutrition clients ensuring the required number of high risk contacts are met.
Collects and evaluates information from the WIC record which can be used for quality improvement purposes.
Makes client referrals to other provider agencies in an appropriate manner.
Maintains records and reports such as the Employee Activity Report (EARS), travel and time sheets in an accurate and timely manner Has a working knowledge of the FL WiSE System.
Attends and participates in staff development, training and required in-service training sessions, as well as regularly scheduled departmental staff meetings.
Other duties as assigned by the supervisor.
Qualifications: Minimum – Willing to work before, during, and/or beyond normal work hours or days in the event of an emergency.
This may include working in a special needs or Red Cross shelter, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural Willing to travel to different WIC sites in Lake County Currently have a bachelor’s or master’s degree from an accredited college or university in nutrition science, community nutrition, clinical nutrition, dietetics, or public health nutrition.
