SUMMARY Serves as a liaison between the professional staff, community, hospital, healthcare facilities, physician, physician's staff, elder care attorneys and elder planning financial advisors.
Acts as a resource for information to ensure quality care for the patient.
ESSENTIAL DUTIES AND RESPONSIBILITIES 1.
Complies with agency policies and procedures.
2.
Ensures open line of communication and education between the agency, physician and physician's staff, hospital and facility staff, attorneys and financial planners servicing our demographic.
3.
Serves as a liaison between the Agency and the referral community.
4.
Conducts medical community awareness to promote services of the agency.
5.
Keeps referral source profiles current.
Provides documentation/information to SVP, Assistant Administrator, Director of Nursing, Scheduling Supervisors and clinical staff to facilitate quality patient care, track referrals and meet goals set.
6.
Retrieves signed POCs or Physician Change Orders from physician's offices as requested.
Notifies Nurse Manager immediately if there are problems obtaining signed POCs/VOs.
7.
If certified, will work as a Geriatric Care Manager, providing GCM services in the patient setting.
8.
Will reach goals for sales calls and admissions.
9.
Participates in staff development activities and in-service meeting as required.
10.
Represents the agency at all times in a professional and loyal manner.
11.
Represents the agency at various community network meetings and events.
12.
Maintains client/patient confidentiality in accordance with Florida Law and the Health Insurance Portability Accountability Act (HIPAA).
13.
Performs other duties as assigned.

LANGUAGE SKILLS Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents.
Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community.
Ability to write speeches and articles for publication that conform to prescribed style and format.
Ability to effectively present information to top management, public groups, and/or boards of directors.
MATHEMATICAL SKILLS Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages.
Ability to apply concepts of basic algebra.
REASONING ABILITY Ability to define problems, collect data establish facts, and draw valid conclusions.
Ability to interpret an extensive variety of technical instructions in diagram form and deal with several abstract and concrete variables.
